I hope this mail finds you well! I'm still in the process of obtaining the DS -2019. Could you please help me to get an invitation letter with Dean Martin's signature for a funding application at my University?

The invitation letter is supposed to be printed in the paper with logo of the University of Michigan. The following information has to be included:

• The contact information of both Sponsor faculty and visiting student 
• The birth date of visiting student(By the way, my birth date is 1988/06/13.)
• The visitor status and the research program name
• The visiting duration
• Whether provide financial support or charge any affiliation, registration, or tuition fees
• The language evaluation results for the program

 Also, the research proposal attached needs to be signed by Dean Martin.  Thank you very much.
